provide visual feedback that "create account" has been clicked #675

From the usability testing:

13. There's no visual indication that you've clicked the
    "Create account" button, so some people double-click.

 There's an easy tweak here:

   - Just add a spinner or "Creating ..." text, which would
     be good practice with all our AJAX calls in any case.
