upgrade to CakePHP 2.3.1 #171

It's possible this might tidy up some general issues, including the superannoying Postgres ordering problem, and the problems I'm having getting PHPUnit installed

Upgrade to 2.2 first -- found no clear statement on the implications of going from 2.1 to 2.3, but migration docs seem to imply just enough material changes on the upgrade to 2.2 to make this necessary (incl. conf changes).

NB Authentication changes may affect MM-FMS htauth behaviour, so be careful.
