Remove all references to migration tool #46

Now that Every Politician exists we not longer need to link to the migration tool. In a future pull request we need to add back in some links to Every Politician.

@chrismytton I added the Migration label to all the tickets related to this Tool I could find this morning — many of those should probably just be closed now.

@tmtmtmtm Thanks for that. I've gone through and closed all of those tickets now. I've also opened https://github.com/mysociety/popit/issues/836 so I don't forget to add links to every politician.
