Add a mechanism to say that amendments have been sent to the ERB #172

This is separate to the main erb approval, because while the initial protocol might have been approved, the amendments might still be awaiting approval.

This is essentially a checkbox (in the admin only) that says "Amendments have been sent to the ERB" as an aide-memoire for the site admins to note that fact.
