No backups will load (with CFG or GX) just get black screen then "Error Reading Disc....." #145

What steps will reproduce the problem?
1.Start up CFG-Loader,
2.Select a game (when i check the debug it appears there is no base on any 
ios??).
3.using ios 222-mload / or 249,250 and try to play the game = Fail.

What is the expected output? What do you see instead?
For the games to load up.

What version of the loader are you using?
CFG USB Loader 68

What is your System Menu version (e.g. 4.2u)?
4.3U

What is your media type (HDD, USB Flash, SD/SDHC)?
HDD

How is your media formatted (wbfs, fat32, ntfs)?
wbfs

Which cios are you using, which version, which base (222,249,...)?
(Please try both 249 and 222 with cfg-222.dol)
249 and I tried 222?

Does it work with other loaders?
No I tried GX and that does not work either. When I run 'pimp my wii' it 
appears that 222, 249 are installed.

Does it work with the latest or an older version?
No

Please provide any additional information below.
GX used to work but when playing golden eye it crashed and has not worked 
since, sorry for being a NoOoOoOoB!!  Please see attached photo.

Try reinstalling cios 249, recommended: d2x v6.
Anyway this is not a loader problem but a setup problem, it's best if you look 
for help elsewhere.
