Form selector: UI improvements #215

In the form selector:
- add a Utils::MiniSplitter between the tree selector and the text viewer
- put "add to current patient" and "add to all patients" in a groupbox
- allow drag'n drop usage between the selector treeview and the current form 
view
- allow removal of sub-forms here???
