Create regular, yellow and red icons for tileset type #193

MassGIS would like a unique icon to indicate the type="tiled_overlay" in the 
folderset. Four white squares that make up a square is fine (like the raster 
icon but one color).  Yellow and red exclamation mark versions are also 
required to handle the out-of-scale and not-available cases.

I'm getting some weirdness with which icons show when.  When I initially open 
the viewer I see yellow "!" for Tax Parcels and Structures, but with the roll 
of parchment picture instead of the little squares picture (see attached 
initial_icon_view.gif.  Then, the Detailed Features looks like the little 
squares picture but it has a red "!" when it's drawn OK.  (is this because some 
tiles are not available?).  Then, if I zoom in to about 1:17,000 scale the 
Detailed Features and the Tax Parcels draw on the map and have the tile picture 
with no "!" which is correct.  Structures still shows the yellow "!" with 
parchment picture.  If I zoom in more Structures looks like a red "!" with 
little squares picture at first until everything draws and then it has little 
squares which is good. If I zoom out the correct yellow "!" icons draw with the 
little squares picture.  

So 2 things:  yellow "!" start out wrong picture, then right themselves.  
Detailed Features is not behaving as I'd like, but it's hard to explain.  Maybe 
we need a screenshare session.

Also, if I zoom in so everything draws on the map and looks like tile.png and I 
drag one tileset above another the two that I didn't drag go from having 
tile.png icon to having the database icon.

If I then pan they all 3 tilesets go back to being tile.png

Sorry, there's just a lot of logic around these icons to figure out! 

Here's another interesting case:  I zoom in so all tilesets draw on the map and 
show the basic tile.png.  I Remove all and they successfully remove.  I add 
back a tileset - while the spinner is spinning the database icon shows, until 
the spinner is done and then the tile.png shows. 

Also, the icons in the "Available Data Layers" area are databases instead of 
tile.png - maybe that is impacting some things.

Detailed Features is not showing red "!" anymore when I first start up - good.
Also no roll of parchment icons when I first start up - good.
Zooming in and out the icons are correct.
Reordering the datalayers the icons are correct.

The one remaining issue I think is when I delete all the datalayers with 
"Remove All" and then I add one back I see the red "!" until it stops drawing.  
I really don't think that is necessary - that doesn't happen for regular 
datalayers.  The spinner indicates it's loading and the red "!" is only to 
display if the loading process fails.
