Abilty to prefill MassGIS geocoding service form values for some fields #220

Paul Nutting suggested a possible enhancement of, if MassGIS custom geocoding 
(red pin button) is ever used for MuniMappers, having the ability to prefill 
the Municipality and State fields for the user.  The code would need to read 
preset values in a config file.
