looks like there is some for of leakage between the bulk head and motor casing. I understand the high temperatures made a hole on the casing but looks like initially some exhaust gases stated escaping from the bottom. Bolts are used as fasteners to couple the bulk head and solid motor so this may be a possible route for exhaust gases to leak through.
