db2epub.py cannnot see value settings.DOCBOOK_XSL #14

 With default install, db2epub.py cannot access DOCBOOK_XSL value in settings.py.
 DOCBOOK_XSL specific as settings.DOXBOOK_XSL from db2.epub.py.
 or import line change like below

> from docbook2epub.settings import *

and 'settings.SPAM' change to just 'SPAM'.
