Text references a "left" and "right" image, however the images are stacked vertically. This change to a "three-col" tag fixes the problem.

where are you seeing them vertically stacked? looks fine in HTML and PDF for me.

I'm using the latest version of Firefox. It loads like this on my laptop (1366x768) and at work (~1600xsomething). Windows 8 and XP respectively.

Seems to load side by side in IE 10 though. Weird.

I'll be using a new system to build the book from now on so am closing this.
