Need to find why and how the duplicate record inserting into lp user's name report.

I have check the function from where record is inserting into lp user's name report, but that function is working correctly. I have check that function goly and varsha as well. didn't get yet from where record getting insert into the report.

I have work with the function and test it, coordinated with ujjwal as well, but didn't found how the data is getting duplicated.

I have change in the report for knowing from where record getting added, for there I understood, whenever students status getting change, for that student data is getting duplicated. I will work on the function.

It's completed, once I will check the report and data is getting store correctly, after I will close it.
