No relative entry point into offset campaigns

Currently there is no way to enroll midway through an offset campaign. For 
example, an offset campaign for pregnant mothers would have different messages 
depending on the stage of pregnancy. If a woman in her fifth month of pregnancy 
is enrolled in the offset campaign, she should not receive nine months worth of 
messages and should be able to receive the relevant campaign messages. Any 
enrollment in an offset campaign, including using a reference date, will start 
the user at the beginning of the campaign and schedule all of the campaign's 
messages.

See the motech-campaigndemo branch, specifically the message-campaign module's 
OffsetProgramScheduler for an example of scheduling offset campaign messaged 
based on an initial offset period. CampaignRequests already have a field for 
initial offset, it is simply not being used by the current campaign module in 
order to schedule messages based on that initial offset.
