1) adding form id "sign-up-form" 2) adding a div that has a class of "form-group" 3) adding the signup CSS link in the head

Great job, me as a reviewer cannot tell which issue number is associated with this pull request.

Usually we add "Resolve #123" in the beginning of the pull request description (not the title). That way the reviewer doesnt have to go and search for what issue this is addressing. Replace 123 with the actual issue number.
