Resolved Issues #2, #4, #12

Resolves #2 and closes #4 through create_epidemiology f(x). The existing db is not changed. Closes #12 during initialization. The existing db is not changed.

Other things

• added disease_uri to the config
• added a good chunk of logging
• completely removed the alert_add_epi code, env, and data dependencies.
• created a new env file.
• I ended up getting locked out of the EpiPipeline API so I could not do the final test on if the epidemiology nodes are actually created. However, the APIs were tested and they work. They only thing that may not work as intended is the node and property construction using Cypher. Hopefully that can be a quick fix if it is not correct.

For other people to optimize if needed:

• Use the batched functions in the EpiPipeline4RD API to speed up the classification and extraction
