verify ontology mappings are correctly handled #182

ontology mappings may not be getting deleted properly, ontologies that are deleted may not have mappings to them deleted, and ontologies are showing duplicate mappings on a regular basis.

Some of these things may be causing some 500 errors when ontology mappings are displayed.
