Clarify permissions needed and explain one issue that can arise #36

I had some trouble setting up llama because it's not obvious what set of AWS permissions the account will require.

This PR adds some more details to the README from the lessons I learned along the way.

Document a sufficient set of permissions for bootstrapping and using llama.
Explain how to track down certain errors in one situation where llama does not provide a useful error message.
