Installed AMP, but Form Model Mgmt doesn't come up; WebScript error #6

What steps will reproduce the problem?
1. Install AMP
2. Create Contraints and JsonModels spaces
3. Restart Alf Comm 4.0
4. Browse to http://127.0.0.1:8080/alfresco/wcs/form-builder/model-manager

What is the expected output? What do you see instead?
I was hoping for the Form Model Mgmt UI but instead get a 404 with this 
exception: 
The Web Script /alfresco/wcs/form-builder/model-manager has responded with a 
status of 404 - Not Found. Message: 02300002 Script url 
/form-builder/model-manager does not map to a Web Script.

What version of the product are you using? On what operating system?
Alfresco Comm 4.0.d, downloaded today along with the FM AMP from Google.  
Windows 7 OS

Please provide any additional information below.

Unusual. Go to your webscript index here: 
http://127.0.0.1:8080/alfresco/wcs/index

In the index there should be a "Form Management" family. If its not there, the 
AMP didn't install correctly.
