Set exchange level based for new 1D2D lines #283

The exchange level of the 1D2D lines is currently based on

the v2_connected_pnt.exchange_level
if that is NULL, the v2_cross_section_location.bank_level (interpolated)
if that is NULL, the DEM value at the 1D end of the 1D-2D

In the new design, the crest_level of crossed levees / obstacles needs to be incorporated. One proposal is:

the v2_exchange_level.exchange_level
if that is NULL, the max of crest_level of all crossed levees/obstacles (= breaking change)
if that is NULL, the v2_cross_section_location.bank_level (interpolated)
if that is NULL, the DEM value at the 1D end of the 1D-2D
