Use full width where page is all tables

Use :page-layout: page-wide in the document header to display the page Contents nav above the content and use the full width for content.

To be used only pages where we have a lot of tables that contain a lot of information, and no other content. For example, configuration settings.

The layout should never be used just to stretch content to a wider space.

TBC - replace page Contents nav with dynamic selections above content.
