Clean up folders #745

This change tidies the repo:

New icon in keeping with branding.
Remove unused CI folder.
Remove top-level issue template (replaced by .github/issue_templates).
Remove defunct test projects.
Remove CHANGELOG as we maintain it in the wiki.
