Fix Planners A to Z table format #89

There was an extra column in the table causing a misalignment issue #88. I removed the extra column and added spaces to make the table more readable, although some lines are still too long due to links. The contents of the table are not modified. Not sure how the atoz.csv is related to the atoz.md table, even their column names are not the same.

.csv dates back to when I was going to write a ruby plugin to auto compile the table from a csv. Now it's included mostly a secondary convenience for me when I make a push to write more of the A to Z.

Okay, just checked the pre-deploy build. Looks good, changes merged.
