Create Answer Survey Form API and Integration

[ ] Create API for Answer Survey Form
- List all corresponding questions for this administered survey and all answers available for each question
- It should also return each question rules, if any
[ ] On click of answer survey form, this API will be triggered
[ ] Implement question type (for now: multiple choice question, multiple answer), question answer layout (horizontal)
[ ] Implement question rules
[ ] On Save & Submit, only allow if all rules are met
[ ] Show response message after submit
[ ] If submission is successful, disable selection of answers. All fields should be on read-only
