On Create Evaluation form, update ff date range picker:

Issue - On Create and Edit Evaluation (Evaluation Period and Evaluation Schedule), once you select start date or end date it will automatically displayed as start date and end date with the same value.

Issue - On Create and Edit Evaluation (Evaluation Period and Evaluation Schedule), once you select start date or end date it will automatically displayed as start date and end date with the same value.

The date appears the same because you clicked on Feb-02 twice. When you click it twice, the library assumes that your start date and end date are both set to that day.
