@emmaacha can pls create mock screen for this and have it reviewed by Sheila.

Once screen is approved, we can start development for this screen. Thanks 🙇‍♀

Initial discussion from standup:

It will be same as View Evaluation Form, minus the list of Evaluations on the side
Screen will contain all the items of the survey (for now, maybe)
Actions will include: Cancel & Exit, Save & Submit
