[ ] Apply these UI changes in Answer Survey Form
[ ] Add more whitespaces between each menu items
[ ] Decrease size of each menu items that can fit at least 6 items per row
[ ] Add Clear button on the other side of Total, once clicked clear current selected items
[ ] Display categories in sidebar
[ ] Add space below Exit and Submit buttons
[ ] Add Php before price
