[x] Add status badge beside survey admin name

If form is submitted:

[ ] On click of Submit, scroll to top to show error or success msg
[ ] Show total amount selected and price of each item
[ ] Hide category sidebar
[ ] Hide Save & Submit button
[ ] Only show selected answer/s (refer to ss below)
