Improve README to clarify that this is a third-party-project #82

In addition to some changes to the design, I'd like to mention that this project is not the official repository (nor a mirror of the official repo) of PJSIP.

Specifically regarding lots of issues and bug reports we get, I think visitors often mistake this as the official source. Therefore, we often have to close issues that are not related to this repo but rather to pjsip itself. That's why I'd consider clarifying (right at the very top, if necessary) this repo's real intent. People who don't read until the bottom of the page won't know otherwise or they make wrong assumptions.
