Add `failedRefresh` to the metadata of a refreshed element. #72

When a readRefresh fails, trace (the failed read metadata) is left in the element's [propNames.metadata].failedRefresh

Coverage increased (+0.1%) to 82.594% when pulling f108ec0e1e9f5080e3875a875025159e15451ef0 on fix/refresh-status into 31a0529001bd3afa473b6934b5bad8df179f0687 on master.
