120 is unrelated, also I cannot seem to figure out how this triggers. Doesn't happen for me. If anyone sees it in action and catches it "on tape" that'd be neat.

Step to reproduce:

Kill with the help of the NPC there all the infernals (several times may be required) Pull the Pit Commander and make the NPC aggro and kill him. When there is respawn you can get a normal or a bugged formation.

Looks like when you clear the area and they respawn bad they also get a bugged aggro range. In fact they run to the NPC right after positioning as if someone pulled them.
