Tested in duels and battlegrounds. It breaks from damage often enough.
No nerf needed.

It depends on the health of your target. Fear breaks after some percentage of damage dealt to the mob. So you can make 10k dmg on feared elite mob and it will still be running around in fear, or one hit to low level mob and fear will break.
The level was reduced in wotlk, so it's normal that players might take more damage before fear breaks compared to other expansions.

Fear doesn't have a threshold at tbc, or at least it wasn't the major mechanic that was regulating it. the 3.1 patchnote was a nerf of the 3.0 formula 43% hp to 3% no heals on one fear https://www.youtube.com/watch?v=lLcazvAGtOM 2:00 if threshold existed it was then 40% mini
On the other hand https://www.youtube.com/watch?v=kk2yfdLOISI 0:10 the lock casts a fear on the other lock, damages on him are corruption, agony, immo and the pet. That fear breaks nearly instantly although lock just lost 2% hp. And it ain't the devour magic of the pet that dispels it, cause it comes a little bit later and it removes the shadow embrace.

Fear seems to stay on target for a long time, even when the target is attacked. I don't know the fear threshold for damage, but it seems really high here.
