Fire left on the ground from the archers in the gauntlet in Shattered Halls are invisible until you take damage from it. Some can stack and just insta kill people.

Same with fire arrow visual, it's occasionally not shown and the fire patch suddenly appears without notice.

Still a problem, seems to be a bit client based since someone can see the fire but someone else can't but can see another fire
