[MNT] Rename app to `digest` and add logo #111

Changes proposed in this pull request:

proc_dash renamed to digest across the modules and README
README reorganized slightly and updated with clearer instructions
Favicon and logo added
Navbar title updated
Switched to lighter color for navbar/buttons to complement logo
