UI Fails to Generate #122

What steps will reproduce the problem?
1. go here: http://myweb.wit.edu/hiness/test.php

What is the expected output? What do you see instead?
 The form generates but The UI never does, is this because javascript is not enabled on my schools web server. The form should generate with the same UI you have on code.google, or am I supposed to apply my own custom css?

What version of the product are you using? On what operating system?

Javascript must be enabled when using pfbc 1.x.  Version 2.x, which is in qa 
currently, will offer improvements for situations where javascript is diabled.
