Remove Rails plugin-related code and documentation #1689

Rails plugins were discouraged when the framework moved to Rails 4.0. We've deprecated Rails 3.2 and have considered removing it in Major Release 9.0 If we decide to remove support for Rails 3.2 in Major Release 9.0, let's remove any code related to installing the library as a plugin and the corresponding documentation.
