Pathpoint UI should point to Github repo instead to encourage customers to open an issue on the repo instead

We need to change this form. This support method is obsolete. I suggest we do the following

• Get rid of the drop downs and edit forms
• Add a message describing how Pathpoint is developed and supported
• Replace submit with 2 buttons
  • Create GitHub Issue
  • Create Discussion Forum Thread
