cookie trail shown at each question level vs. top of group #131

What steps will reproduce the problem?
1. display a multi-question group
2. observe that the group captions cookie-trail is displayed at each question

What is the expected output? What do you see instead?

Should display the cookie trail once on each folio, not at the per-question 
level.

Please use labels and text to provide additional information.
