Migrate issue templates to issue forms

Description

Sine issue templates are regularly ignored, we should give the github issue forms a try.

Let's do this. There have been a number of completely unstructured reports lately. They have the same info, but it's just more time consuming to go through them.

This repo was migrated fully, Mail bug reports as well. The other Mail templates and other repos still need to be migrated.
