Add the connector and channel names to all server log errors/warnings #2247

We need to go through all connectors and add the connector/channel names to all logger/alertController outputs so that users will be able to tie all of the server log errors to specific channels and connectors.

Issue has been solved by replacing log appenders via a custom library: http://www.mirthcorp.com/community/forums/showthread.php?t=221641
