Updated registration form items (including waiver) for Fruit Picker

Describe the bug We need to update our registration form. If someone is returning to an existing account, they need to be prompted to fill out this info. They should not be allowed to move forward without filling out all required information.

To Reproduce Steps to reproduce the behavior:

1. Go to '...'
2. Click on Fruit Picker
3. (https://portal.notfarfromthetree.org/register?account_type=fruit_picker)

Expected behavior The registration needs to include the following information for all accounts:

First Name Last Name Preferred Pronouns Birthday Email Phone Number Street Address Street Address - Line 2 City Province Postal Code Emergency Contact First Name Emergency Contact Last Name Emergency Contact Relationship Emergency Contact Phone Number Emergency Contact Email

Demographics Please share a bit about your background and identity. This information is used to help us see who we are serving and tell our story to potential supporters.

(make a checkbox for these)

1. I identify as BIPOC
2. I identify as 2SLGBTQIA+
3. I moved to Canada within the past five years
4. I moved to Toronto within the past five years
5. I have children
6. I do not have children
7. I identify as low-income (link to photo) or do not make a thriving wage in Toronto ($25.05)
