Error: layers are added several times in feedback tool #39

How to reproduce: 1) select feedback button (the dialog appears) 2) close feedback dialog 3) select feedback button again (the dialog appears again) 4) check the active checkbox of any layer, the feedback adds this layer twice.

Also if the user fails the captcha it gets two error messages
