Error exporting data to MGCP fgdb/shp #543

Found this in the soon to be pushed regression smoke tests (see #534), mass-nh.child, port-au-prince.child, and samoa.child (see commented out lines referencing this issue:

Error running osm2ogr: Integer value is not a valid enumeration and bounds are not set. 3

Re-enable related lines in mass-nh.child/Makefile and samoa.child/Makefile; also re-enable the port-au-prince.child after fixing.

@mattjdnv Do you mind taking a look at this when you have a chance. It may just be something silly I'm doing but also may be related to some production errors we're seeing now.

Actually, this isn't related to the problem in production but still probably worth looking at.
