Test Drive V-Rally : missing backgrounds, 2d/3d priority issues

Test Drive V-Rally has some priority issues with 2d and 3d.

On the title screen, only the options are visible. There should be background 
graphics. See pic and comparison video (1:35 onwards):

During a race the 3d elements overlap the 2d HUD elements in some places, most 
obviously seen on the pause screen (see pic).

Tested on r78, NTSC GDI version.
