Catchment lookup by year #26

At the moment, the trust catchment from the final available year is applied to all years to estimate ICS data from organisation data. Where new Acute Trusts have opened, it isn't appropriate to apply the latest year's catchment to each organisation as the make up of who attends the trust will be different prior to the trust opening.

Also, review whether this step needs to perform the calculation each time, or can a look up table simply be created.

Also, consider filtering for different organisation roles in the look up process so only patient facing organisations are included.

The issue is that Isle of Wight NHS Trust opened in 2012. This is the only change to Acute Trusts since 2011. This will cause an issue for "unknown" organisations in 2011 that are close to IoW NHS Trust, because they will then only identify one "known" trust to adopt the catchment of, rather than dividing their catchment between the 2 nearest trusts.

None of the modelling goes back to 2011, so this issue does not affect what the project is doing
