Add contributor guidelines and code of conduct #45

Attempted to address comments raised in #30. Conscious might be overkill, but hopefully useful to start a discussion, as well as to repurpose across NHS-R content.

One outstanding issue is that currently anyone can create a branch and a PR, so it is technically incorrect to state that you have to fork the repo- I'll raise this as a separate issue.
