Properly incorporate the NHS-R vision repo #19

The vision files are all separate .md files. Do they exist anywhere as one document? If not, we can look into compiling them into one

I'm going to answer my own issue- it's probably best to just supersede the NHS-R vision document and move the material inside this repo, rather than allowing them to get out of sync with each other. I can take care of this, the mess is my fault after all 😳

Great, thanks. If you make it just one md / Rmd doc, it will keep the script of the bookdown Rmd cleaner, but no worries otherwise. You could use the same approach I used in the bookdown script to compile it quickly!
