Create a GitHub template for new 'discussions' #220

What

Similar to the template that was created when creating an 'issue', we should look to create one for when people create a new 'discussion' which includes:

  1. What
  2. Why
  3. Anything else

Why

This will help guide people when they create a 'discussion' to ensure that their suggestion is clear for others.
