eRS and GP Connect boundaries #127

Need to clarify boundaries and use cases which apply to eRS (e.g. Any-to-any) and GPConnect appointments capability

To date eRS does not require appointment booking into GP surgeries and is therefore out of scope and there is no overlap between the two services
