Common resource population guidance #403

There should be resource population guidance for resources common access the spec - Patient, Organization, Location, Practitioner, PractitionerRole.

Also for the Appointments resources.

Similar to that done for the Structured resources:

@james-answer as well as adding in a new page(s) for common guidance we said we would review the main resource guidance page too. Can you look at what this might need changing please.
