Immunisations – notgiven in profile #978

Fixed value for ‘notGiven’ removed from profile but still present here: https://developer.nhs.uk/apis/gpconnect-1-3-2/accessrecord_structured_development_immunization.html#notgiven

This reflects that we have only added imms not given to the v1.5.0 specification as it was deferred when immunisations was curated for 1.3 so the profile and guidance for 1.3 is that the ‘notGiven’ would be fixed as “false” but that may not matter if only v1.5.x goes into production.
